• Pager number
• Name of pager holder
• Whether or not the pager is being returned physically or if it has been lost/stolen

When returning the pager physically also use the following instructions:
Take it to the pager office (ED2 North, Rm 5102-1) and leave it at the desk, with or without a note. They can also, send it via the inter-campus mail to Pager Office, Mail Stop F408.
